That thing with the set screws looks like a 5" to 6" adapter. It isn't an Axeman part, just heave it. Soak the screws down with penetrating oil and give the screws a good smack with a hammer, should be more cooperative after that. Hit them square, you don't need more work.

The Pope's Hat is just a piece of pipe with a flange welded onto it. The pipe goes down in about 8", the flange bolts on top of the boiler and about 3" sticks out the top. 5" stove pipe fits right over it. You should run 5" til you get near the base of the chimney, then step it up to a larger size.
